You are here: Library Media ServicesFormsMedia Services Request FormsUse this form to ask Library Media Services to reserve an item in advance FOR SPECIFIC DATES, or to put an item in our collection on reserve. To put a personal item on reserve, you must use the Reserve List.
Reserve ListUse this form to put a personal item on request. This is the same form that is used to put books on reserve. It is your responsibility to make sure that the items are available, and are not missing or lost. The Reserve List is available only in PDF format; print out the form before filling it in.
Purchase Request FormsUse this form to ask Library Media Services to purchase an item. Please note that while all requests will be considered, there is no guarantee that an item will be purchased.
Before you submit a purchase request, please note that the option to preview a media item exists. We can arrange for a preview of a video before you officially make a request for purchase. Previewing an item is especially recommended for expensive items which you know little about or have not viewed before. The preview policy is as follows:
If you would
like to request that a preview item be purchased, please fill out a
Purchase Request Form. Remember to include
a brief statement as to why you think it would be a valuable teaching
tool and asset to the collection. This will help our librarians make
purchasing decisions.
