Students and parents: You can securely fill out and submit the required Immunization and Health History Form online. Find instructions and a link to the form here.
COVID-19 Safety Protocols
We are committed to providing the highest level of care and ensuring your health and safety as well as our team’s health and safety.
We have implemented measures recommended by the Centers for Disease Control, American College Health Association (ACHA), Illinois Department of Public Health and other agencies. The following are some of the measures designed to provide the safest level of care possible.

We continue to monitor the CDC guidelines for best practices and to ensure a safe and comfortable environment for you.
Counseling
We offer confidential counseling services to enrolled students to help them adjust to college life, address psychological issues, and maintain a healthy lifestyle.
Student Health Services
The Wellness Center provides basic health care services to all registered students, including preventive care, treatment of minor illnesses and injuries, immunizations, lab work and more. All of our services are confidential, and many of them are free.

Fees and Payment Options
Wellness Center visits are free of charge. We charge a nominal fee for medications, medical supplies and lab work. You may pay those fees with a credit/debit card or personal check, or you may put the charge on your student account.
After-Hours and Emergency Care
